Submitting an application does not by itself reserve a seat. A seat is reserved only when the required deposit has been received and the applicant receives a written confirmation from Preeminent NDT LLC.

Seats are assigned on a first-paid, first-served basis. If a class fills before a deposit is received, the applicant will be offered the next available class date or placed on the waitlist for that class.

Preeminent NDT LLC reserves the right to decline any application, including applications that do not meet the published prerequisites for the selected course.

Tuition for each course is shown on the course listing. A non-refundable deposit in the amount shown for that class is due at the time of application.

The remaining balance is due no later than seven (7) calendar days before the first day of class unless a written payment arrangement has been approved in advance.

Students whose balance is unpaid on the first day of class may be denied admission to the classroom and may forfeit their deposit.

Purchase orders from employers are accepted at the discretion of Preeminent NDT LLC. The student remains responsible for tuition if the employer does not pay.

Tuition covers instruction, course materials, and examinations administered during the class. It does not cover travel, lodging, meals, third-party certification body fees, or retest fees unless expressly stated.

Cancellations received more than fourteen (14) calendar days before the first day of class: tuition paid above the deposit is refunded in full. The deposit is non-refundable but may be applied to a future class within twelve (12) months.

Cancellations received seven (7) to fourteen (14) calendar days before the first day of class: fifty percent (50%) of tuition paid above the deposit is refunded.

Cancellations received fewer than seven (7) calendar days before the first day of class, or after class has begun: no refund is issued.

One transfer to a different class date is permitted at no charge if requested at least seven (7) calendar days before the first day of class. Additional transfers may incur an administrative fee.

Preeminent NDT LLC may cancel or reschedule a class due to insufficient enrollment, instructor availability, weather, or other circumstances. In that event the student may transfer to another class date or receive a full refund of all amounts paid, including the deposit. Preeminent NDT LLC is not responsible for travel or lodging costs.

Training hours are documented for certification purposes. Students must attend all scheduled classroom and laboratory hours to receive a training certificate.

Students who miss more than ten percent (10%) of scheduled hours may be required to make up the missed hours or repeat the course at additional cost.

A student who does not attend and does not cancel in writing before the first day of class is recorded as a no-show and forfeits all amounts paid.

Written and practical examinations are administered as part of the course where applicable. Passing scores are set by the governing written practice or specification.

Training completion certificates document attendance and instruction hours. Certification to a Level I, II, or III is issued by the employer under its own written practice, or by the applicable certification body, and is not granted by Preeminent NDT LLC unless expressly contracted.

One retest may be offered at the instructor's discretion. Additional retests may require a fee and rescheduling.

Students are responsible for providing current near-vision and color contrast examination records where required by the applicable specification.

Students are expected to conduct themselves professionally, follow all instructor directions, and respect facility rules and other students.

The use of alcohol or unlawful substances on the premises is prohibited. Any student who appears impaired will be removed from the class without refund.

Cheating, sharing examination content, or falsifying attendance or experience records results in immediate dismissal without refund and cancellation of any certificate issued.

Preeminent NDT LLC may dismiss any student whose conduct disrupts instruction or creates a safety hazard. Dismissal for conduct is without refund.

Students must supply and wear the personal protective equipment listed for their course, including at minimum safety glasses and closed-toe protective footwear, during all laboratory activities.

Students must follow all posted safety rules, equipment operating instructions, and instructor safety directions at all times.

Students must immediately report any injury, near miss, equipment damage, or unsafe condition to the instructor.

Students working with ultraviolet light, chemicals, or magnetizing equipment must follow the applicable handling, ventilation, and skin-protection instructions provided in class.

Radiographic courses may include the review of radiographic film and, where applicable, observation of radiographic operations. Students acknowledge that ionizing radiation is hazardous and that all radiation safety rules, barricades, and instructions must be strictly obeyed.

Students must disclose to the instructor before class if they are pregnant or may be pregnant so that alternate arrangements can be made for any activity involving radiation sources.

Where required, students must wear the dosimetry provided and return it at the end of the course. Dosimetry must not be shared, altered, or deliberately exposed.

Students may not operate radiation-producing equipment or handle radioactive sources unless properly licensed and expressly authorized in writing.

Students acknowledge that nondestructive testing training involves the use of industrial equipment, chemicals, magnetic fields, ultraviolet light, and in some courses radiation, and that these activities carry inherent risk of injury.

To the fullest extent permitted by law, the student releases Preeminent NDT LLC, its owners, employees, instructors, and agents from liability for injury, illness, loss, or property damage arising from participation in training, except where caused by gross negligence or willful misconduct.

Preeminent NDT LLC is not responsible for personal property brought to the training facility.

Preeminent NDT LLC's total liability arising from any course is limited to the amount of tuition paid by or on behalf of the student for that course.

Course materials, presentations, examination content, specimens, and written practices provided during training remain the property of Preeminent NDT LLC or its licensors.

Materials are provided for the student's individual use. Reproduction, distribution, recording, or resale of course materials or examination content is prohibited without prior written permission.

Photography, audio recording, and video recording in the classroom and laboratory require the instructor's prior permission.

Preeminent NDT LLC collects the information on this application in order to evaluate eligibility, administer training, document instruction hours, issue certificates, and maintain training records as required by applicable specifications.

Training and examination records are retained for the period required by the applicable specification and may be provided to the student, and to the student's employer where the employer is paying tuition or is the certifying employer.

Student information is not sold. Information may be shared with certification bodies or auditors when required to verify training.

Students may request a copy of their own training record by contacting Preeminent NDT LLC in writing.

By submitting this application the student certifies that all information provided, including education, prior certification, and experience, is true and complete.

Electronic acceptance of these terms, including a typed signature, has the same effect as a handwritten signature.

These terms may be updated from time to time. The version accepted at the time of application governs that enrollment.

These terms are governed by the laws of the state in which the training facility is located.
